We would like to congratulate Virginia Sizemore on completing our Safe transition program and returning home with family. Mrs. Sizemore is a 84 yr old female who was transferred here at the Harlan Health and Rehab Center due to her inability to Ambulate long distances and perform her essential ADLs to return home safety. When she was admitted she required min assist to perform her ADLs and mobility tasks and unable to walk over 10ft.
Thru skilled therapy services and our safe transition program Mrs. Sizemore was able to increase her strength, ROM, balance, safety awareness, and ADL performance. She was able to ambulate up to 200ft with a rolling walker or hand held assist.
